An interactive, hybrid, learning experience to discuss key aspects of the life-long care of a patient with Congenital Heart Disease (CHD) and the challenges of transition from paediatric to adult services. This meeting is aimed at both paediatric and adult cardiologists including trainees, consultants, and clinical nurse specialists from both the ACHD and PH teams.
